Certain universities and colleges with mathematics graduate programs
explicitly state or are reputed to be particularly interested in
producing graduates who are qualified in both teaching and research
(as opposed to primarily research, though almost every program will
have a token comment about teaching).  I am interested in a list of
such universities and colleges and any available reports or other
documents pertaining to this topic.

Examples that I am aware of:
