I want a list of all:
1) the math text books for K-12(including author,publisher and
address) and which of these text books are adopted by what State in
the US as of SY 2000-2001.
2) Elementary(K-6), Middle School(7-8) and Secondary(9-12) public and
Private schools in all the 50 States, their addresses.

I might suggest splitting these two questions.  The first question is
fairly detailed by itself.

In the 2nd question, there are more than 85,000 public schools alone,
not to mention private schools.  Google researchers should be able to
find you list sources and their cost.

For question number 2, I did not actually expect to be given the list
of the schools but rather be pointed to accurate database sources or
websites containing them from which I could them request a service or
report/s. I actually expect 90% of the work in #1. You can drop the
second part of the question and I will post it separately.

I was stunningly unsuccessful in getting what you needed, even though
I thought that it would be an easy matter to get the approved textbook
lists from state departments of education.  This is one resource that
you might consider:

Not all states adopt at the state level; some states leave it up to
the district or school to decide. Would a list from those states where
the adoption is at the state level be acceptable?
