I want the death date and death place of a relative as follows; 
Charles Albert Hartman.  He was born on April 31, 1875 in Salt Lake
City Utah.  He married Dorothy Wesch September 26, 1922, in Yakima
Washington.  They may have lived in Wisconsin.  Charles was named
Charles Albert Hyde at birth, but changed his sir name from Hyde to
Hartman later in life.

Considering April has only 30 days, are you sure of the date of birth?

We must assume a type on the birth date, and use April 30

Tibiaron and GA researchers:

Hartman, Charles A. does not show up in the Social Security Death
Index with that birth date.

Considering the birth date could be in error, this is my best guess:

Charles Hartman 
Birth:  unknown 
Death:  Nov. 4, 1935 
Burial:
Wood National Cemetery *
Milwaukee
Milwaukee County
Wisconsin, USA
Plot: 28, 246

certificate from the state of Wisconsin here:
