Bruce,

I got yet another bit of circumstantial evidence, and the cumulative
effect gives me enough confidence that I feel I can post as a
definitive answer.

A web site of a Howard Stern fan has a 10/24/97 log entry describing a
visit by KNB Effects to the radio show, and they present Howard with a
copy of the prosthetic member used by Wahlberg in the movie.

The company is listed in the White Pages as KNB EFX Group:
7535 Woodman Place 
Van Nuys, CA  91405

I checked, and the phone number is indeed valid.

I'm glad the answer was exactly what you needed! I am not a specialist
in the area of movie-making. I just happened to have a random piece of
knowledge that worked out for this particular question. Knowing that
the work in question would fall under the realm of "special effects
makeup" gave me the lead I needed to track the rest down. Why do I
know that? I don't know; maybe from watching the movie F/X too many
times?

At IMDB, there are 3 people credited with Special Makeup Effects on Boogie Nights:
James Hall
Louis Kiss
Robert Maverick

and Howard Berger is the special effects makeup supervisor.
