Microsoft Word allow users to insert checkboxes, drop-down menus and
form text boxes into documents.
To insert a checkbox:
1) Open the Forms toolbar by clicking View>Toolbars>Forms
2) On the toolbar, you will see an icon for checkboxes (that looks
like a checkbox!), clicking on it will insert a box at the location of
your cursor
3) To edit these fields, you must use the Form Field Options button on
the Forms toolbar. This is the forth icon from the left (it is an
image of a hand pointing to a form). This allows you to format your
checkbox and design its size, default value and associated function.
To insert a radio button:
Word does not have a feature that allows users to directly insert a
radio button. I looked into their technical support documents and
nfortunetly, found no references to radio buttoons in Word documents.
However, you can still have a feature that would be similar to a radio
button by inserting an image, or by going to 'Insert' and clicking on
the 'Symbol' button. This will bring up a number of special characters
from which you can choose (the symbol closest to a radio button
appears in the second row). Another option is for you to simply copy
and paste a radio button from an HTML web page, and then associate a
ActiveX function to it.
