How likely would a surgeon be to castrate someone for non-medical
reasons, upon request? (for oneself, of course) Drugs (if a recurring
expense) are not an option.  Is there a permanent "chemical
castration" akin to the sort sexual predators receive that is
available to the public?

Requested procedure is not in relation to gender reassignment; rather,
Matthew 19:12 KJV.

Already well researched on consequences. Effects far outweigh minor
health problems, and yes, those are very minor.  Consideration of such
a procedure has been weighed for approx 2+ years without
discouragement.

The only cirumstance I can think of in which a physician would be
willing to remove undiseased testicles is gender reassignment (sex
change) surgery.

I agree with pinkfreud- that would be the only circumstances. 
Certainly no surgeon would do it just on the whim of the patient- they
would probably be reccomended to have a psychological evaluation. 
Also, gender reassignment subjects generally undergo months of
psychological and non-surgical physical treatments prior to an
operation.  So simply "upon request" with no questions asked would
just not happen.

Practices might vary depending upon country.

There is definitely a doctor who can help you.  There was an article
in Philadelphia Magazine about a year or two ago about a doctor in
Philadelphia that performs male castrations for men who wish to escape
their sexual desires.

Before undertaking castration be sure that you understand the full
ramifications of that action.  Removal of the testicles means a body
will no longer be able to produce it's own testosterone.  Testosterone
does much more than make you horny. It helps you build muscle and bone
mass as well as produce body and facial hair.   If a person has his
testicles removed he will need to take hormone supplements or else
face serious health risks such as osteoporosis and heart disease.
