I am trying to locate a template which I can use as the basis of
"Service level Agreement" for a "Managed Security Service Provider"

Will you only be accepting free templates for your answer? I have
found books on the subject,  but the cost triple digits, so I highly
doubt that this information is available for free on the internet.

I am looking for a working document that can be rapidly adapted to our
needs. Assuming this is MSSP specific then yes I would be prepared to
purchase. I would also be prepared to look at "terms of service" for
Managed Firewall services.
