I'm an foreign medical graduate (FMG), and planning on coming to the
US to do an Emergency Medicine residency. There are approximately 130
residencies in the USA.
Some residency programs (or their host medical schools) will only
accept FMG's on a J1 visa (which has to be sponsored by ECFMG). Others
will sponsor FMG's themselves for an H1B visa, either routinely or for
exceptional circumstances. Sometimes this decision is made at the
level of the residency director, sometimes there are medical school
rules regarding Graduate Medical Education (GME) that override any
decision that could be made at residency level.

I would like a list of emergency medicine residency programs in the
United States that are willing to sponsor an H1B visa for an FMG, and
an indication of the requirements for H1B sponsorship for each of
these programs (I expect that only a minority of programs will sponsor
an H1B visa, so this level of detail should be manageable I think).
The answer needs to clearly distinguish between cases where there is a
LACK of information available for a program and those where there IS
information indicating the type of visa(s) that may be sponsored. For
those with no online information regarding visa type, I would like the
name, phone number & email of the relevant contact person at the
residency program who would be able to answer such a question. I would
be willing to pay an additional $200 for a search where these programs
are contacted by phone/email to get the relevant information, such an
answer would need to be fully referenced regarding who the information
was obtained from, their position at the residency and the time /
nature of the communication. If such contact is made by phone/email,
it may be worth asking about both a POLICY regarding H1B sponsorship,
as well as asking "Has the program sponsored a candidate in the past
for an H1B visa?@

I have identified all the schools offering an emergency residency and
separated them into three categories:

--Schools that have the H1B sponsorship
(one or two of these I have also emailed to confirm)

--Schools who do not make the information available on their websites
(I have emailed all of these schools)

--Schools that do not accept the H1B visa
(this is from either their websites or from their response emails)

is fully approved and accredited.  It should also state whether a
candidate from that school would have a position available to him/her
on their return to their country of origin.  It should also state
whether the candidate would have the support of the ministry should
they require a visa.

These requirements are in place simply to assure that anyone entering
this residency program has the background and ability to successfully

sole discretion and in conformity with the guidelines set forth below,
sponsor H-1B peititions for foreign nationals who seek
employment/appointment at the UFCOM.

This visa permits only the activities normally associated with the
title or appointment. This visa is available to any professional with
at least a Bachelor's degree who will engage in the practice of the
profession for which s/he was trained and which requires at least a
Bachelor's degree. This visa is issued in 3-year increments and is
valid for a maximum stay of six years.

The following conditions, in addition to regular H-1b guidelines, must
be met for prospective clinical faculty who have graduated from
medical school outside of the United States and who will be performing
direct patient care:

The alien doctor must have passed the Federation Licensing Examination
(FLEX) parts I & II). Equivalent examinations designated by the
Secretary of Health and Human Services include the National Board of
Medical Examiners certifying examinations (parts I, II, and III) and
the United States Medical Licensing Examinations (steps 1, 2 & 3).

   1. He or she is competent in oral or written English, as
demonstrated by passage of the English language proficiency test given
by the ECFMG (Educational Commission for Foreign Medical Graduates),
which currently sponsors foreign doctors for graduate medical eduation
in the J-1 category;
   2. He or she has a full and unrestricted license to practice
medicine in a foreign state or he or she has graduated from a medical
school in a foreign state; and
   3. He or she has a license or other authorization required by the
state of intended employment to practice medicine.

H-1B VISAS FOR CLINICAL RESIDENCIES AND FELLOWSHIPS

Unlike the J-1 Exchange Visitor visa, the primary focus of the H-1B
visa is employment, not training.  Individuals enrolled in clinical
residency and fellowship programs are enrolled in educational training
programs and therefore, except in unusual circumstances benefitting
the institution, the most appropriate visa category for these
individuals at the UFCOM is the J-1 rather than the H-1B visa.

The H-1B visa category may be used for clinical residencies and
fellowships in graduate medical education (in lieu of J-1) under the
following limited circumstances:

       1.  Where the foreign national is a graduate of a U. S. medical school; or,

       2.  Where the foreign national currently holds an H-1B for
clinical graduate  medical education (at another institution): or;

       3.  For competitive reasons in exceptional circumstances (guidelines below).

In all cases involving clinical residencies and fellowships in
graduate medical education where use of the H-1B visa category is
contemplated, the foreign national must be H-1B eligible at the time
of appointment. and there must be sufficient time for the UFCOM campus
administrative offices and the University of Florida International
Faculty and Scholar Services (UF/IFSS) to prepare, and the United
States Citizenship and Immigration Services (USCIS) to process and
approve the H-1B petition before the start of the program.

Other conditions include the following:

     *  The H-1B category will be used only where the program con be
completed within the six-year time limit of the H-1B.

     *  H-1B visas are prohibited from outside employment/moonlighting.

TRAVEL COSTS & EXPENSES

H-4 dependent visa fees, required photos, border crossing fees, the
cost of travel to/from a U. S. Consular post abroad and to the Uniter
States, cost of lodging, meals. etc. will be the sole responsibility
of the H-1B beneficiary (to the extent permitted by law).

AUTHORIZED AGENTS OF UNIVERSITY OF FLORIDA FOR H-B PURPOSES

The H-1B petition requires sponsorship by an employer.  Only
representatives of the UF/IFSS are authorized to sign all documents
submitted to the USCIS and other state and federal agencies in which
University of Florida is identified as the employer/petitioner. Any
petitions submitted without the prior authorzation and signature of a
designated representative of the UF/IFSS may result in a request to
the government for official withdrawal of the petition.

Exception Criteria

The following factors shall be considered in deciding whether to offer
exceptional H-1B sponsorship to an IMG cadidate for clinical
residency/fellwoship:

     *  The IMG is judged to be a strong future prospect for Attending
Staff appointment;

     *  The IMG is a medical school honors graduate or otherwise
achieved an outstanding GPA;

     *  The IMG achieved an outstanding score/percentile on the USMLE;

     *  The IMG has professional achievements, recognition, awards,
publications, etc, significantly above those of his /her peers.

Procedure

The following procedure will be used in considering a request for
exceptional H-1B sponsorship of an IMG.

1.   IMG submits request to appropriate Program Director with
explanation and documentation addressing the above Exception
Critieria;

2.   Program Director reviews Exception Critieria to assess IMG's
request and decides whether to request/recommend H-1B sponsorship;

3.   Program Director recommends exceptional H-1B sponsorship to the
Director of Graduate Medical Education (DGME);

4.   The DGME will approve or deny the exceptional H-1B sponsorship
request, and communicate that decision to the Program Director.

?	F-1 visas, accompanied by Optional Practical Training authorization

Residents and fellows are not permitted to train under any visa

qualified and meeting all State Department and Iowa licensure
requirements.  Additionally, the parent department of the training
program must document its approval prior to the commencement of
processing of any kind of visa; no department is required to accept
trainees with visas.

Questions concerning this policy should be brought to the GMEC.  All
information pertinent to a trainee?s visa, including but not limited
to the trainee?s qualifications, departmental approvals, and
acknowledgments, must be filed with the House Staff Affairs Office at
UIHC."

Hi em_doc,

I have more to add to the list of those that do sponsor visas.  I have
asked all the programs that have replied in the positive what their
requirements are.  Some have responded and some have not.  I have
given you all the current information I have on the programs.  There
were only two schools that seemed to really put barriers on the
visas-- Wright State University really pushed the J1 and Cincinnati
requires the explicit approval of the Graduate Medical Office.  I hope
this helps you in your search!

In addition, in my emails I did ask whether the programs had sponsored
any applicants in the past. I have indicated when a residency
coordinator informed me that they had sponsored these visas in the
past. Only one, UCLA, explicitly told me that they have a current
student on an H1B visa.  If you want to know more information about
the UCLA program (I don't even know what country you would be
concerned with or what tests you have taken or anything so I'm no good
to talk to them specifically) call Julie Zamoyski at (310) 825-7050.

Great work so far. I would just like to clarify one thing: The school
/ residency itself doesn't sponsor J1 visas, but once an IMG is
eligible for residency and is offered a slot, the J1 visa sponsorship
by ECFMG is just a formality.
On the other hand, H1B visa sponsorship has to be done by the
residency / school, and this is why some schools are reluctant to
sponsor H1B.

In some of the information you've listed, the answers received refer
to the program sponsoring 'visas'. I'm concerned that the school's may
be confused between whether you're asking if they ACCEPT IMG's on
'some kind of visa', or whether their response is specifically
regarding H1B sponsorship BY THEM. I would like to be sure that where
the school/residency is listed as 'sponsoring visas', this related to
H1B sponsorship specifically.

I will submit payment for $200 for the initial search right away, and
look forward to submitting the additional $200 once the remaining
information is back. Thanks+++, great work.

Hi em_doc,

In my emails to all the programs, I specifically asked about the H1B
visas.  I asked "Does your residency program sponsor these visas? If
so, have you sponsored one in the past and what are the requirements?"

Most of the residency coordinators that wrote back to me replied
something along the lines of "I'm sorry, we do not sponsor any visas."
Just to make things faster, sometimes I said, "does not sponsor visas"
when really what I should have said was "does not sponsor H1B visas"
or "does sponsor H1B visas".  Where I have listed a college as
sponsoring the visas-- the seven in the original answer and then next
seven in the clarifications-- they have specifically written on their
web sites or responded to me that they accept the H1B visas. Where I
have said that they do not take visas, it means that they accept
either only the J1 or have told me that they don't sponsor the H1B.  I
hope that clears things up a bit. If you need any more help, just let
me know.

I have some more additions to make today, by the way, and this time
they're all positive!  The coordinators must have been gathering
information to send.

They don't have any minimum score requirement that they post, and
current residents' scores range from 205 to 245.
There isn't a cutoff date after graduation. They accept and review all

All applicants have to be registered with the NRMP.
They have 36 residents in the program and 3 are foreign medical graduates.
You would have to have your ECFMG certificate for your application to
be considered complete.

The residency coordinator was very helpful and very willing to respond

They do require that international graduates have at least one year of
training in a US medical school or hospital prior to acceptance.
Along with the application, they require three letters of
recommendation, preferably from emergency medical physicians, your
medical school transcript, dean's letter, board scores, cv, personal
statement, ECFMG status report if applicable and an optional photo. 
They want to note that University Medical Residents Service is a
for-profit agency and thus is subject to an H1B visa cap.

They have never and do not currently have anyone on H1 visas.  They
contacted the immigration attorney of the school and the attorney
recommended the information found here:

That makes 17 total that have responded positively.

Sorry about not seeing your other request!  

Hmm, I'm not sure about the tipping policies exactly. If the box only
allows you to tip $100, then I suppose that that is what is allowed. 
You could simply do that and then if you need any more clarification
on your answer or any other questions answered, you could ask another
question-- which I'd be willing to answer, of course!

Paying for the answer doesn't terminate the question at all-- I have a
few other questions which I'm helping to clarify for the askers after
the completion of payment and everything.  So I'll still be here if
